The SafeStart Accident Incident Investigation Enhancement Series is a reality-based training program— a series for enhancing your workplace’s incident investigation process. The enhancement comes from incorporating the SafeStart concepts and techniques into existing investigations.

By getting people to look for the SafeStart state to error risk patterns that are involved in most incidents and injuries, this program sheds some light on root causes (human errors) and provides a whole new perspective on what can be done so the incident is not repeated.

 

Formal incident investigations are typically conducted when a serious injury or an incident with high potential takes place. This training teaches and motivates employees to conduct their own investigations on close calls and small errors they make, so they can learn from these and avoid the big ones.

The focus is to prevent similar incidents, not just the same incident, from occurring again. Often, incident investigations result in an engineering or procedural “fix” that will prevent the exact same incident from occurring again, but won’t necessarily prevent similar incidents.

For example, we need to help people get better at “moving their eyes before they move their bodies”, in addition to creating a better shelving system for jagged metal.

 

The focus of the program is on help, not blame. It not only identifies what traditional safety corrective actions are required, but also prompts people to identify the applicable SafeStart Critical Error Reduction Technique(s).

All the incidents used in the training are real. The people interviewed are the people who were involved in the incident.

The training session is 4 hours and is designed primarily for those who conduct incident investigations at the workplace (typically supervisors).

However, it can also be conducted for all employees on site.
