SafeStart Principles

The Four Critical Errors

The principles of SafeStart are based on an initial survey of 20,000 people (since then verified by more than 2 million people) which found 85 to 95 per cent of accidental acute injuries were caused by people making one or more of four critical errors while doing something:

  • mind not on task
  • eyes not on task
  • being in, or getting in, the line of fire
  • losing balance, traction or grip.

Caused By Four States

It was also found that when they made these critical errors they were in one or more of four states:

  • rushing
  • frustration
  • fatigue
  • complacency.

SafeStart teaches this 'state to error' pattern of human behaviour and helps people develop critical error reduction techniques to reduce the risk of injury in any situation – at work, at home or on the road.

Critical Error Reduction Techniques (CERT's)

SafeStart teaches four techniques to help people avoid making critical errors. Participants in the SafeStart programme are trained in these four Critical Error Reduction Techniques as the final step in their journey toward error and injury prevention.

Fatalities in Australia

Each year...

About 440 people are killed in incidents at work.

Around 1500 people die on the road.

An estimated 2000 to 4000 die at home or in the community, with 350 of those children.

About 9000 people die as a result of medical mistakes (based on a Harvard University study conducted in 2000).

SafeStart is a Human Error Prevention Program that has achieved a consistent
60-90% reduction in workplace incidents within 6-12 months of its implementation.

Almost 2 million people have been trained in SafeStart at more than 2000 organisations (7000 sites) in over 40 countries.
